Thank you for sponsoring a gift for an adult in our Adult Day Health Care (ADHC) Program
The participants who are eligible to receive a gift attend one of our Adult Day Health Care Programs which provides healthcare and recreation for people with vision loss as well as chronic medical conditions. Thanks to a generous donation, we’ve covered the cost of clothing for the adults during the holiday. Your contribution will go toward providing needed personal care items that can be distributed to clients with dignity throughout the year or in the case of the gift packets for their birthdays.
As a sponsor, you can donate towards a gift package or provide a gift.

We gladly accept your financial donation for the purchase of a gift package or individual personal care items for an ADHC participant.
Provide a Gift
Please complete the application below to secure your registration to provide a gift. As a sponsor, you are asked to provide a gift package that contains an age/gender/ability-appropriate gift item.
We will contact you shortly to provide the gender of the participant you have sponsored.
The deadline for gift packages is Monday, December 15, 2025. Gifts must be brand new and can be wrapped in themed paper or placed in a decorative gift bag. With your confirmation e-mail, you will receive additional information including suggested gift items and how to label your gift.
