2025 Tax Preparation Program

For Individuals with Vision Impairment and Blindness

Lighthouse Guild continues to serve people who are visually impaired by helping with filing their 2025 federal, state and city income tax returns through a partnership with the IRS, the FourLeaf Federal Credit Union and Urban Upbound. 

Tax agents will be available at Lighthouse Guild (located at 250 West 64th Street) on the following days: 

  • Monday, February 23, 2026, from 9:30 AM to 4:30PM
  • Tuesday, February 24, 2026,from 9:30 AM to 4:30PM
  • Wednesday, February 25, 2026, from 12:00 PM to 7:30PM
  • Thursday, March 26, 2026, from 9:30 AM to 4:30PM
  • Friday, March 27, 2026, from 9:30 AM to 4:30PM

An appointment is required to reserve time with a tax agent. Appointments are available on a first come, first serve basis. To make an appointment, please contact Lighthouse Guild’s Access Center at 646-874-8683, Monday through Friday between 9:30 AM and 5:00 PM. Please arrive 15 minutes prior to your appointment and check in at the Lighthouse Guild Lobby Reception Desk. 

Appointments are limited to 60 minutes. The tax agents will do their best to expedite your return as quickly as possible, but you may experience a brief waiting time. 

This service is limited to basic income taxes with standard deductions generally for people who made $70,000 or lower in 2025. See following pages for specifics of what will/will not be prepared and documents you will need to bring.

Important personal documents that you need to bring with
you include:

  1. Picture ID for you and your spouse (if filling jointly, both must be present)
  2. Social Security card for you and your spouse (if filing jointly) or Individual Taxpayer Identification Number (ITIN) documents for you, your spouse, and all dependents.
  3. Birthdates for you, your spouse, and all dependents.
  4. A copy of last year’s tax returns (for reference)
  5. Proof of income (Forms W-2, 1098, and 1099)
  6. Information for other income
  7. Proof of health insurance (Form 1095-B or 1095-C)
  8. Information for all deductions/credits
  9. If applicable: total paid to daycare provider, their name, address, and tax ID number
  10. For direct deposit of refund: proof of account number and routing number
  11. Certificate of legal blindness or letter from your eye care physician to attach to the federal form (If you are filing for the first time, you may obtain a certificate from the New York State Commission for the Blind.)

To make the best use of your time with the tax agent, please check to see that you have all your items prior to your appointment and before submitting your documents to the tax agent. It will also help if you arrange your documents in logical groupings. 

Lighthouse Guild Tax Preparers will prepare: 

  • Acceptable forms: W2, 1099-Int, 1099-Div, 1099-G, SSA-1099, W2-G, 1098-T, 1095-B or 1095-C, 1099-R (only if taxable amount determined on Box 2a)
  • Types of Income: Wages, salaries, etc. (Form W-2), Interest Income (Form 1099-INT), Dividends Received (Form 1099-DIV), State Tax Refunds (Form 1099-G), Unemployment Benefits (Form 1099-G), Gambling Winnings (Form W-2G)
  • Certain Tax Credits: Earned Income Credit, Credits for Qualifying Children & Other Dependents (Sch 8812), Child & Dependent Care Credit (Form 2441, Education Credits (Form 8863)
  • Limited Types of Income: IRA Distributions (Form 1099-R) limited, Pension Income (Form 1099-R) limited, Social Security Benefits (Form SSA-1099)
  • Basic tax returns only
  • Standard deductions only
  • Only Federal and New York State Tax Returns

Lighthouse Guild Tax Preparers will not prepare: 

  • No outside of NY income
  • Certain Pension & Retirement Income without taxable amounts determine on Box 2a (1099-R, RRB-1099, CSA-1099)
  • Certain Social Security Benefits (Form RRB-1099)
  • Self-Employment Income: Cash Based, Uber/Lyft, Ride Share, Food delivery services, etc. (1099-Misc, 1099-NEC, 1099-K)
  • Sale of Home (Form 1099-S)
  • Schedule A “Itemized Deductions”: 1098 Mortgage Interest, Real Estate Taxes, Personal Property Taxes, Medical & Dental expenses
  • Cancellation of Debt (Form 1099-C)
  • Schedule C with loss, depreciation or business use of home
  • Schedule D “Capital Gains and Losses”: Crypto, E-Currency Investments, Sale of Stocks (1099B)
  • Credits for Sick Leave and Family Leave for certain Self-Employed Individuals (Form 7202)
  • Form SS-5 (request for Social Security Number)
  • Form 8606 (non-deductible IRA)
  • Form SS-8 (determination of worker status for purposes of federal employment taxes and income tax withholding)
  • Marketplace Health Insurance (Form 1095-A)
  • Parts 4 & 5 of Form 8962 (Premium Tax Credits)
  • Forms 8615 & 8814 (children’s unearned income)
  • Form 2210 Underpayment of Estimated Tax
  • Form 9465 Installment Agreement
  • Forms 8379 Injured Spouse Allocation
  • Foreign students and scholars income tax returns
  • Form W-7 application for Individual Taxpayer Identification Number (ITIN)
  • Returns with casualty/disaster losses
  • Prior year and amended tax returns

Please note that these guidelines for what will or will not be prepared is applicable to for our slots in February with FourLeaf FCU. Slots in March will be with Urban Upbound, who may be more able to assist in greater needs for people with income as high as $97,000.
